Patient Transfers

Mt. Washington Fire & EMS will make every attempt to facilitate requests for Non-Emergent Transfers. However, Mt. Washington Fire & EMS places priority on 911 calls for service, and the ability to have resources available to respond. A Transfer Request should be requested at minimum of twenty-four (24) hours in advance. Requests for Non-Emergent Transfers must have the following prior to performing the request:

• Patient Demographic Facesheet
• Physician Certified Statement (PCS)
• MWFEMS Transfer Request Form
